I have never tried PHP Surveyor, so this is just an idea, but if you have tried sending it to yourself, perhaps spamassassin checks your emails for spam (if spamassassin is enabled) and mistreats these emails. Have you tried white listing the email address that these emails goes to?

Joomla will let you create a member section (ie the visitor must have a username and pw) by default, to have them pay for this you would need some sort of addon. I would set up a joomla test site and go to extensions.joomla.org and see if there are an addon that will let you do what you want. Another note on joomla... I know I recommended you joomla in another thread. Since then version 1.5 have been released, which is much more simple to use and I believe that it will let us users create much better joomla sites than we used to be able to do (more professional, better looking etc).

While I haven´t found anything explicit about DodosMail, I have found out through a google search that others have had problems with other Dodo scripts not being php5 compatible. I know there are others here that have used or use DodosMail, so hold on and someone may know the answer to that.
