That's why I don't recommend using the .htaccess file and use the .shtml extension.

2 things to check 1. In the admin of security & privacy you must have the radio button in the (low) security settings to allow images to be posted set to "yes" 2. Make sure you set the uploads folder to 777 permissions

I believe if you take the ErrorDocument 404 out of the .htaccess file and name the error document with a .shtml extension you can put the document in the particular folder you want the error to display for. So if you have a 404.shtml in your public_html folder and then want a different one in you sub domain or other folder just create a different 404.shtml file and place it there.

Have you checked to be sure you have entered zeroes and "o"'s correctly. That could cause problems. I know it sounds dumb but I've done it.

You need a trailing slash on this line. $folder = "http://www.samisite.com/uploads/"; And the only folder that requires 777 permissions is the uploads folder. Also, the page should be able to function from any path in your domain.
