Adding Web Form Data To Database

I am currently trying to design a page that contains a form that a user will complete. I would then like the data from that form to be added to a database. I would also like to be able to query the database for specific information.

 

Unfortunately, I am very new at this, and have absolutely no idea where to begin. Can anyone suggest a good web site that contains a tutorial, or any other place that might help me get started.

I can make it for you only send me the information that you want to collect and i ll make for you a php script and mysql database for free.
