jnull Posted January 6, 2005 Posted January 6, 2005 Hi ... I am looking for a way to take a paper form, create it on a web page, and when submitted, have it returned to me as a form. EXAMPLE: The IRS 1040EZ form, I want that exact form on a web page where I can fill in the information, and then click SUBMIT, and I receive that form in an email completely filled out. Any suggestions where to go to get a tool like this? Quote
TCH-Bruce Posted January 7, 2005 Posted January 7, 2005 Don't know of something like that but you can create forms in Adobe Acrobat that can be filled out and emailed. I found this information site about creating forms. http://www.aces.edu/ctu/techref/software/acrobat/ Quote
woesap Posted January 7, 2005 Posted January 7, 2005 J null, I am not sure abt making the form the way it looks on paper. I guess you need hand coding for it (other than Bruce's suggestion). I you have a form then I think Ultimate Form Mail can help you further. The've got a version where you can put your form also into a webmaster template(cutsomize it) and put the variables into the fields. Thus you get exatly the same form with the data mailed to you. If you need further assistance or have questions abt the possibilities of UFM. They have an excellent support forum Quote
