Phplist Problem

Not sure when this started, but anyone signing up for my newsletter gets this error message:

 

Sorry, sending the email to request your confirmation failed, please click "Reload" to try again. If it still does not work, it may be because you are listed on our "Black List", which means that you cannot receive emails from our newsletter system. In that case, please contact the administrator.

 

Their email gets added to the phplist, but no confirmation email is sent so it is impossible for them to confirm their subscription.

 

Any ideas why this is happening?

 

It was working previously, but I only noticed this problem today.

I've taken a look, but am reluctant to change anything.

 

As I say, it was working fine with people able to sign up. Without me changing anything this problem has arisen.

I've now replaced it with a temporary form to collect sign-ups, but if anyone knows why this is happening let me know.
