Using Mysql To Populate Pdf Forms

Is anyone using a database or even html form data to populate fields in a pdf file? I've read some things about FDF, but I think it is server-side and needs to be compiled. If it's not part of the existing server config, I doubt that it can be used.

 

Any suggestions/comments?

 

Basically, I've got a web app for a client that collects the form data and stores it in a mysql db. They are going to need to have the ability to print a completed standard document pre-filled with the db data.
