New To Php

Ok Im trying to install a database program for a game a Play. A working on ecan be found here:

The instructions which are loaded on my server can be found here:

I can get to Upload the Roster Files

 

Which looks like this per the instrucitions:

However when I get to this section

Upload Character and Guild Data (Users with CharacterProfiler and GuildProfiler) It informs me that I can go to this link:

 

http://facciolli.com/wow/Roster/HTML/admin/updateMembers.php

 

And get an "upload" screen. Except when I go to that updateMembers.php page I get quite a few errors that I have no idea what they mean.....

 

If someone could look at that page with the errors and give me a general idea of what I should be looking for it would be appreciated. Again ive followed the instructions which seemed pretty straight forward...

I´m a little confused on what page you see the errors.

If I go to http://facciolli.com/wow/Roster/HTML/admin/updateMembers.php I see some text, an empty text area and a submit button.
