File Upload Through Html

Me and another member struggled to get html file upload up and working through this tread http://www.totalchoicehosting.com/forums/i...wtopic=8560&hl=. I had everything working, and tested it several times. I got a message from one of my users this morning saying that it was not working now. I had not made any changes! I don't know what happened.

 

Here is the error recorded in the error log.
