Before doing a fresh installation (which seems a bit drastic), have you tried fresh download and unzipping of the MT 3.15 upgrade package? Sometimes files can get garbled in unzipping (at least with winzip).

I've been running a weblog with Movable Type 2.64 for many months, without any problems. For the past couple of months I've been unable to upload images as thumbnails. The error message: "Thumbnail failed: Reading image failed: Exception 430: Unable to open file ()" I ran mt-check.cgi and apparently have all modules installed (but I'm not exactly an expert, so maybe I'm missing something). Any ideas? Thanks!
