Php Sending Forms.

I am trying to create a form in my phpnuke site. I tried adapting my html form to my phpnuke site and it shows up nicely but when I click to send the information in the form it does not work. (Guess its not that easy )

 

What I am trying to do is create a simple form were my user enters his name, email, and other information and that information is sent to my email.

 

Can some one PLEASE help me with the code.

i found a way to do it.

 

i took the feedback moduel and modified the code to work for me in the way i needed.
